    To sign up for Online Banking, fill in the application below, print, sign and mail to 9600 Bellaire Blvd., #252, Houston, Texas 77036 or fax to 713 414-3663.  In a few days, you will receive two letters containing Login ID and temporary password separately.  Use this Login ID and password to log in to Online Banking.  The first time you log in, you will be asked to change your  password.
